Agent Swarm Failed Tool Calls

In a swarm, failed and rejected calls get receipts too, so you can see exactly which agent broke and where.

Why failures need receipts

Each recorded call can be opened as its own receipt. The receipt lets a reader inspect the captured tool name, status, time and output commitment that are available on that record. Failed and rejected calls should remain visible as their actual status.

Use the receipt checker to compare the saved receipt data. That comparison checks the record, not the truth of a claim inside it.

Rejected versus failed

Preserve the returned state for every attempt. A rejected or errored call should be labeled as such, with its own receipt when available, and should not be counted as completed work.

Explain the next action without hiding the failed step.

Finding the first failure

  1. Start from the result or failure the user noticed.
  2. Walk backward through the workflow receipts in call order.
  3. Identify the first call whose status or output differs from what the next step expected.

Report the observed status and the next check without describing an unverified cause as fact.
